Parents,
Sorry to bother you with another form, but the district (correctly) wants to ensure that when we drive the kids to a meet, all the cars are safe, insured and driven by a licensed driver.  There is a form here that the district requires all drivers to submit.  If you could please complete this form you can bring it to Ceci the first time you drive for the team, or get it to Ceci when you see her, or have your athlete bring the form to the coach.
